Conditional Approval, Minor modifications

In case of resubmission with minor modifications, following documents should be submitted to the ERC office by the relevant deadlines. Your request will be reviewed at the next available meeting, provided it is complete.

1. Covering letter containing a summary of modifications and page numbers
2. One (1) hard copy of complete proposal including Participant Information Sheet (PIS), Informed Consent Form (ICF) and data collection tools with the date and version number in relevant languages ( with changes highlighted and updated date and version number)
3. Copy of latest ERC communication
4. ERC checklist

Resubmissions with major modifications

In case of resubmission with major modifications, following documents should be submitted to the ERC office by the relevant deadlines. Your request will be included in the agenda for the next available ERC meeting, provided it is received by the relevant deadline and is complete. 

1. One (1) hard copy of covering letter containing summary of modifications and page numbers
2. One (1) hard copy of complete proposal including PIS, ICF and data collection tools with changes highlighted and updated date and version number
3. One (1) hard copy of latest ERC communication
4. ERC checklist
